Transitional care may help reduce rate of rehospitalization among chronically ill patientsMany studies have revealed high rates of hospital readmissions among some populations of Medicare beneficiaries. Recent statistics suggest that as many as one in five patients returns to the hospital within a month of being discharged.

In order to reduce the rate of rehospitalizations, the Patient Protection and Affordable Care Act has proposed numerous initiatives designed to improve outcomes and reduce medical costs for chronically ill Americans. Researchers from the University of Pennsylvania School of Nursing believe that focusing on transitional care can be an effective way to help patients stay healthy and avoid readmission.

The team published a paper in the current issue of the journal Health Affairs, in which they presented a range of solutions that may help providers reduce avoidable patient rehospitalizations and costs.

Among their suggestions was the use of nurses who personally work with patients, family caregivers and health teams to prevent medical errors from being made and improve continuity of care.

"The good news is that available evidence provides those responsible for implementing community-based care transitions programs, accountable care organizations and other models with a strong foundation upon which to build these programs," said lead author Mary Naylor.
