Study explores ways of reducing rehospitalization rate among elderly patientsA recent study conducted by researchers at the Hebrew Rehabilitation Center has found that the number of elderly patients who return to the hospital after discharge has risen significantly over the years. The paper, which appears in the current issue of the Journal of the American Geriatrics Society, revealed that more Medicare beneficiaries are experiencing rehospitalization within 30 days of leaving.

Lead author Randi E. Berkowitz and her colleagues compared patients' discharge disposition from a skilled nursing facility (SNF) before and after the implementation of a initiative that combined palliative care, root-cause-analysis conferences and standardized admission templates.

The team found that the rate of rehospitalization dropped from 16.5 percent to 13.3 percent, a reduction of about 20 percent, after the program was established. Discharges to home rose from 68.6 percent to 73 percent, while discharges to long term care services decreased from 13.8 percent to 11.5 percent.

"The change in discharge disposition observed between the two periods, we believe, reflects an improvement in patient outcomes," Berkowitz explained. "Specifically, a lower acute transfer rate likely reflects improved processes of care in the SNF."
