How to Replace a Medicare Card

Lose your Medicare card? It happens. With you can order a replacement Medicare card. To do so, select the Order Card button located on the Replacement Medicare Card tab from the My Account page. You can also replace a Medicare card by calling Social Security at (800) 772-1213 or by using the online my Social Security account. Select the "Replacement Documents" tab and then select “Mail my replacement Medicare Card.”

Replacement Medicare cards can only be sent to the address of record on file with the Social Security Administration. Please make sure that your address on the Replacement Medicare Card tab is your current address before ordering a replacement Medicare card.

Please note, it may take up to four weeks for you to receive your new Medicare card.

If you choose to receive an entitlement letter by checking the box located on the Replacement Medicare Card tab from the My Account page, you will receive an entitlement letter within 14 days of your successful replacement Medicare card request.

For your protection, you will not be able to order a replacement Medicare card if one has been ordered in the last 30 days.

If you need a Medicaid card, please contact your state Medicaid office.

What does the Medicare card look like?

The red, white, and blue Medicare card has important details about your Medicare health insurance. The card shows your name, claim number, date coverage starts, and whether you have Part A (Hospital Insurance), Part B (Medical Insurance) or both.

Why do I need my Medicare card?

Your Medicare card is your proof that you have Medicare health insurance and is shown to your medical providers.
